Basic Principles

Chordia Therapeutics Inc. (hereinafter referred to as the "Company") considers it as one of its corporate social responsibilities to ensure that the personal information it deals with shall be handled safely and fairly in compliance with laws and regulations, guidelines established by the government and other norms regarding the handling of personal information, and as such the Company shall handle the personal information appropriately in accordance with the following basic policies.

(4) Purposes of use of personal information regarding customers, consumers and patients

  • Communication regarding and response to consultations about medicinal products, etc.
  • Response to and surveillance of adverse reactions to medicinal products, etc.
  • Provision of information and after-sale service for medicinal products, etc.
  • Communication with manufacturing and selling operators, manufacturers, and sellers of medicinal products, etc., and healthcare professionals, etc.

(5) Purposes of use of personal information regarding applicants for employment with the Company

  • Employment screening, notifications regarding employment screening, etc. as well as personnel and labor management, etc. after hiring decisions

(10) Purposes of use of personal information acquired in relation to the research activities of the Company

  • Utilization of the base sequence, which is considered to be an individual identification code, acquired from a commercially available human-derived cultured cell strain or human-derived cell strain or a human-derived specimen of a subject obtained through a clinical study and disease-related information, etc. for drug-discovery research or academic research

(12) Purposes of use of personal information stored in information systems used by the Company

  • Analysis of data, including personal information stored in the information system used by the Company, to prevent, detect and likewise deal with any misconduct within and outside the Company

4. Disclosure and provision of personal information to third parties

The Company shall not disclose or provide any personal information to third parties without consent of the subject of such personal information, except:
(1) if required by laws and regulations;
(2) if necessary to protect the life, limb or property of any person;
(3) if specifically necessary to promote the improvement of public health or the healthy development of children;
(4) if necessary to cooperate with government bodies or local public authorities or any person entrusted thereby in fulfilling their duties required by the laws and regulations;
(5) for outsourcing, business succession or joint use; or
(6) if otherwise exceptionally pursuant to the Personal Information Protection Act.

5. Response to a request for disclosure, etc.

If requested by the relevant person to disclose, correct, add to or delete, or suspend the use of personal data held by the Company, the Company shall respond to such request within a reasonable period of time and a reasonable scope in accordance with its rules.

6. Matters relating to the safety management measures

To prevent any personal information from being divulged, lost or destroyed and to manage personal information appropriately, the Company shall clarify employees’ responsibilities, develop internal rules, educate and train employees, and take physical and/or technological measures and other safety management measures to prevent the divulgence of personal information or other incidents. 7. Information desk for inquiries and management of complaints

The Company shall accept all opinions, questions, complaints or other inquiries regarding the handling of personal information, etc. via its inquiry form. The Company shall appropriately, in good faith and promptly respond to any inquiry, complaint, etc. about the personal information received by a relevant information desk.
